LUNGSOD NG QUEZON Kasabay ng pagdadalamhati ng Philippine National Police (PNP) sa pagpanaw noong Oktubre 20 ni PMGEN Jose Ma. Victor DF Ramos na isa sa mga lulan ng Bell 429 na nag-crash noong Marso 5 sa Laperal Cmpd. sa San Pedro City, Laguna,  ay ang paglabas ng PNP ng transparency report ng kasong grave misconduct laban sa piloto ng helicopter.

Ito’y sunod sa naging rekomendasyon ng Special Investigation Task Group Bell 429 sa pamumuno ni PLTGEN Guillermo Lorenzo Eleazar na sampahan ng kasong administratibo at kriminal ang Pilot-in-Command na si LTCOL Roel Zalatar base sa naging resulta ng imbestigasyon na hindi ito nagsagawa ng risk assessment bago ang takeoff, kawalan ng situational awareness at evaluation sa paligid, at pag-underestimate sa kakayahan ng aircraft.

Lumabas din sa imbestigasyon na airworthy ang helicopter at walang problema ang mga makina nito.

Sinampa na ang kasong administratibo na Grave Misconduct (Reckless Imprudence Resulting in Serious Physical Injuries, Multiple Less Serious Physical Injuries and Damages to Property sa ilalim ng Art. 365 ng Revised Penal Code) sa PNP Internal Affairs Service noong Setyembre 2. Pinag-aaralan pa ang kasong kriminal na isasampa sa nasabing piloto.

Nagrekomenda rin ng safety measures ang investigation group upang hindi na maulit ang insidente.
